 磁共振弥散张量成像评价特发性失嗅 脑白质微细结构损害的研究

  摘要 目的应用磁共振弥散张量成像DTI技术研究特发性失嗅患者脑白质微细结构的改变方法
16例特发性失嗅患者患者组16例年龄性别相匹配的健康志愿者对照组进行 SniffinStick嗅觉功能测试 及脑 DTI检查DTI生成的各向异性分数FA参数图上选择双侧梨状皮层眶额皮层海马岛叶邻近白质 和内囊为感兴趣区ROI),计算每个 ROIFA 采用独立样本检验比较患者组和对照组间 ROIFA 值差 异并与 MMSE评分进行相关性分析结果患者组双侧梨状皮层眶额皮层海马及岛叶邻近白质 FA 值低于 对照组0.05),而双侧内囊 FA 值两组间比较差异无统计学意义0.05)。结论特发性失嗅患者存在嗅 觉皮层邻近脑白质微细结构的损害对于特发性失嗅的发病机制及早期干预具有重要的意义

  Abstract ObjectiveTo investigate the cerebral white matter microstructure in patients with idiopathic olfac
tory loss using diffusion tensor imagingDTIMethodSixteen patients with idiopathic olfactory loss and sixteen normal subjects matched by age and sex were recruited in this study.SniffinStick olfactory test was performed to evaluate the olfactory function of all subjects.We acquired diffusion tensor images with an echo planar imaging
EPIsequence from all subject on  3Tscanner.The fractional anisotropy FAimages were performed using DTIstudioand bilateral Piriform cortexOrbitofrontal cortexHippocampus and Insula cortex adjacent white matter and Capsula interna were delineated as the region of interestingROI),the FA for each ROI was calculat
ed.Independent sample  test analysis was used to compare the FA value of all ROIs between the controls and pa
tients.In additioncorrelation analysis between FA value and MMSE score in patients were conducted.Result
Compared with the controlspatients showed significantly decreased FA value in the adjacent white matter of bi
lateral Piriform cortexOrbitofrontal cortexHippocampus and Insula cortex0.05.There is no significant difference of FA value in bilateral Capsula interna between two groups0.05ConclusionThe patients with idiopathic olfactory loss show the damage of white matter microstructure in the olfactory centerwhich could be important for the pathogenesis study and early intervention of idiopathic olfactory loss.
